This form is a centralized space for Student Affairs and Campus Life staff to submit accolades, achievements and recognitions for inclusion in the Annual Impact Report.

Please consider submitting assessments, projects, or events that you'd like to highlight. You may also submit a particular achievement of a staff person or group, such as an award, degree, or publication.

The information may also be published in other UC San Diego Student Affairs and Campus Life publications.

This form is not for submission of your annual assessment report. You may, however, submit parts of your assessment report you want highlighted.
